Key Partners
Partnerships are a key component in helping the Health & Healing Hub fulfill its vision of uplifting the Central neighborhood by focusing on the root causes of poverty and addressing critical community needs. The Hub’s Cleveland Central Promise Neighborhood initiative team acts as connectors between the partners and residents to ensure resources are leveraged and accessible. Our network of planning, program and community outreach partners will continue to grow as additional organizations join this valuable work. Current key partners that address social drivers of health include:
A Place 4 Me: provides direct services to youth and young adults experiencing homelessness, as well as working to prevent and end youth homelessness. 
Beat the Streets: students participate in wrestling and improve literacy skills. 
Care Alliance: high-quality, comprehensive medical and dental care, along with patient advocacy, referrals to addiction medicine and other health-related services. Federally Qualified Health Center (FQHC) 
Case Western Reserve University School of Medicine: education partnership that includes the establishment of two permanent endowments in addiction medicine.
Cleveland Clinic – Be the Boss: a school-based mental health and wellness program.
Cleveland Metropolitan School District
(CMSD): providing a variety of support at CMSD schools, including Marion-Sterling School. 
Cleveland Central Promise Neighborhood (Initiative of the Sisters of Charity Health System): coordinates and connects residents to wrap-around services that are provided in the Health & Healing Hub with focused efforts on breaking the cycle of poverty by working to improve outcomes for children, families and the community. 
Cuyahoga Community College (Tri-C): a summer program partner helping to develop a macro academic strategy.
Esperanza: expertise in education, mentoring and wraparound support. Provides STEM training through a summer programming partnership.
Family Connections: replication site in Central for the SPARK Kindergarten Readiness Program operated by the Early Childhood Resource Center, a ministry of the Sisters of Charity Health System; supports parent training through parent cafes, kindergarten readiness and home visits.
Fast Break: innovative initiative to increase math skills using basketball.
Foluké ARTS: students learn creative skills to enhance overall literacy skills.
Friendly Inn Settlement, Inc.: key neighborhood partner, teaching healthy eating skills to 40 children weekly, summer programming and arts.
Goodwill Industries of Greater Cleveland and East Central Ohio, Inc.: offers workforce development programming in Central with plans to build a full-service neighborhood grocery store with access to fresh fruits, vegetables, meats and more; plus plans to build a Goodwill Opportunity Center on the Hub campus to provide more employment and career services, plus additional health and social services, while housing other nonprofits collaboratively.
Habitat for Humanity: partnering with Sisters of Charity Health System to purchase and rehab multiple homes in Cleveland’s Central neighborhood.
Joseph & Mary’s Home (Ministry of the Sisters of Charity Health System): medical respite for adults experiencing homelessness, filling a service gap between hospital and shelter for a way back to health and forward to housing.
Leadership Cleveland: leadership development group helping advance the work of the Hub and engage residents through volunteer opportunities and more. (community outreach partner)
Making a Difference: co-designer with the Sisters of Charity Health System on summer programming that focuses on health and wellness, college and career development, goal setting, and more.
PNC: student financial literacy training. Marion-Sterling students learn and earn through curriculum, meeting objectives and student savings accounts.
Rid-All: partnership to teach urban agriculture to Central youth through a garden at Marion-Sterling School and summer programming.
Rosary Hall, Outpatient (Ministry of the Sisters of Charity Health System): dedicated team of physicians, counselors and social workers providing intensive outpatient programming (IOP), medication assisted therapy (MAT), and aftercare and ongoing support.
St. Martin de Porres High School: partner in providing students to help with programming in Central as part of their learning experience.
Taste-ify: provides nutritious meals to Marion-Sterling students.
The Centers: will offer residential detox and in-patient behavioral health services. Federally Qualified Health Center (FQHC)
The Sr. Ignatia Gavin, CSA, Center for Addiction Medicine (Program of the Sisters of Charity Health System): community gathering spaces, chapel, hosted meetings, learning sessions and community events.
Women Religious Archives Collaborative (WRAC): preserving the legacy of women religious in the United States. (community outreach partner)
